Patient Information

Dispatch You are called to the lake foreshore at the Riverton Family Fun Day. A bystander has pulled an 8-year-old boy from the lake after he was submerged for approximately 3–4 minutes. He is breathing but unresponsive. (Noah Papadopoulos, DOB 14/03/2016)
Patient Noah Papadopoulos — 8yr (26kg)
Incident History Pt fell from a pontoon ride into the lake and was not noticed immediately. Pulled from the water by an adult bystander after approximately 3–4 minutes submerged. Bystander states the child vomited once on the bank. Lake water is known recreational water — contamination status unknown. Pt was in the water for approximately 10 minutes total including partial submersion before rescue.

Initial Rapid Assessment

Response Pain
Airway Partially soiled — vomitus and lake water visible in oropharynx. No stridor. Airway at risk due to reduced conscious state.
Breathing Laboured and irregular. Audible gurgling. RR 28 in 30s. Mild subcostal recession. SpO2 84% on room air.
Circulation Rapid weak central pulse. Radial pulse difficult to palpate. Skin pale, cold and wet to touch. Lips cyanosed. CRT 4s.
Disability GCS 9 (E2V2M5). Not orientated. Withdraws to painful stimuli. Pupils equal and reactive to light — 3mm bilaterally.
Exposure Clothing soaked and cold. No visible external injuries. Skin universally pale and mottled. No rash. Core temp appears significantly reduced — shivering absent.
